By completing this form you accept the following terms and conditions - please read them prior to finalising the booking.
Registration Terms
1. All costs and fees are expressed and payable in Australian Dollars (AUD).
2. All fees quoted herein include Australian Goods and Services Tax (GST) of 10%.
3. A tax invoice will be issued following receipt and clearance of payment. The invoice will be sent to the email address provided during registration.
4. Bookings WILL NOT be confirmed without payment. Attendees must either complete credit card details or transfer the fees into the nominated conference account.
5. Attendees are advised that the organisers will not accept responsibility for any loss or injury howsoever caused. Attendees are strongly advised to obtain insurance against property loss, personal injury, travel, registration and accommodation cancellation costs and any other risk.
6. English is the official language.
7. All programmed sessions, tours, functions and events are subject to minimum numbers being reached and force majeure.
8. Credit card charges will appear on your statement as Carillon Conference Management Pty Limited.
9. All personal information collected for registration for the conference will be handled in accordance with the Privacy Policies of Carillon Conference Management.
10. Bookings cannot be shared. Once name bagde will be provided with each registration.
Cancellation of Registration and Refunds
1. Cancellations of registrations must be made in writing to the PTSD Forum 2015 Secretariat admin@ccm.com.au via fax, post or email. Cancellations or changes will not be accepted over the phone.
2. Cancellations received up to 60 days prior to the Forum will be refunded less an administration charge of $25. No refunds will be issued for cancellations received after 30 days prior to the Forum.
3. Up to one (1) substitution is available up to 30 days prior to the Forum without charge. Substitutions notified after this date will incur a charge of $25 (per person).
Insurance
Fees do not include personal, travel or medical insurance of any kind. Attendees are advised when registering for the conference and booking air travel that a travel insurance policy should be taken out to cover risks including (but not limited to) loss, cancellation, medical costs and injury. The Forum and/or the Forum secretariat/organisers will not accept any responsibility for any delegate failing to insure.
Photography
Any photographs taken by the Forum photographer(s) during the Forum may appear on social media and in Forum related publications during and after the Forum . If you do not wish to appear in such images please notify the photographer onsite or advise the staff at the registration desk.
Communication
All communication regarding the Stand Tall Gala Dinner will be directed to the attendee named on this form. All written communication (such as, but not limited to, confirmation letter and tax invoice) will be sent by email, in accordance with conference's sustainability policy.
Privacy
Your privacy is important to us. Your personal information will be used by the conference in accordance with our privacy policy and your privacy preferences as expressed in this form. In addition to the statements in our privacy policy, personal information collected from conference delegates may be shared with corporate partners - the Forum sponsors - so that they may contact you directly. By submitting this form, you consent to us using and disclosing your personal information for this purpose.
